Active Injury Management after Pediatric Concussion (AIM)

Purpose
A systematic approach to the evaluation and management of concussion and evidence-based recommendations for early activity could improve the quality of life for many children. This randomized clinical trial of active rehabilitation and quality improvement initiative will inform and improve the care of pediatric concussion in the emergency department.

Acute neurobehavioral predictors of risk trajectories following youth violence victimization (PATT)

Purpose
To assess the impact of violence on youth outcomes utilizing fMRI and machine learning we will enroll youth victims of violence and follow their outcomes longitudinally. Analysis will focus on protective and negative associations on mental health outcomes including PTSD, Anxiety, and Depression.

Addressing Food Insecurities in the Emergency Department

Purpose
Initiative in the CW emergency department designed to screen and address food insecurity in families. The initiative includes incorporating food insecurity questions into the emergency department workflow, providing families with community resources, and connecting families in need of additional assistance with the Family Resource Center.

Azithromycin Therapy in Preschoolers with a Severe Wheezing Episode Diagnosed at the Emergency Department (AZ-SWED)

Purpose
The primary objective of the current study is an evaluation of the safety and efficacy of Azithromycin (AZ) for the treatment of wheezing and lower respiratory illness in children ages 2-5.

Bedside Exclusion of Pulmonary Emboli in Children without Exposure to Radiation (BEEPER)

Purpose
The purpose of this multi-center study is to identify a set of rules that can be used to exclude a diagnosis of pulmonary embolism in children. The study prospectively examines the sensitivity and specificity of the PERC-Peds criteria as well as the diagnostic accuracy of d-dimer.

Development and Testing of a Pediatric Cervical Spine Injury Risk Assessment Tool (C-SPINE)

Purpose
Multi-center prospective observational study of children who have sustained blunt trauma and may be at risk of cervical spine injury (CSI). The investigators aim to develop and test a pediatric cervical spine injury risk assessment tool that can be used by EMS and ED providers to determine which children warrant spinal precautions and cervical spine imaging after blunt trauma.

IMPROVE: The Effect of Emergency Department and After-Emergency Department Analgesic Treatment on Long Bone Fracture Outcomes

Purpose
A prospective multi-center, longitudinal comparative effectiveness study that capitalizes on the known variability in pain treatment to determine the most effective analgesic regimen for children with fracture pain. The long-term goal is to improve the treatment of injured children. The overall goal of the study is to evaluate and provide evidence for both ED and post-ED pain treatment for children with fracture-related pain.

Pediatric Self-Reported Pain Outcomes via Texting following Emergency Department Discharge: How Does it Compare to Parent鈥檚 Perception?

Purpose
For children ages 8-17 years old with long-bone fractures who are discharged home from the ED, we will compare the short-term pain outcomes reported by children using text messages to pain outcomes reported by their parents to comprehensively describe the outpatient pain experience, an area that is currently lacking.

PROMPT BOLUS: Pragmatic Pediatric Trial of Balanced versus Normal Saline Fluid in Sepsis

Purpose
This is a randomized clinical trial aimed to determine the clinical effectiveness of balanced crystalloids vs normal saline in children resuscitated for septic shock as a part of usual clinical practice.

Sickle Cell Disease Treatment with Arginine Therapy (STArT Trial)

Purpose
The study is a phase 3 randomized, placebo-controlled trial of IV arginine vs. placebo in patients with sickle cell disease age 3-21 years who present for acute management of vaso-oclusive pain episodes requiring parenteral opioids. Children’s Wisconsin is 1 of 10 clinical sites in the US that will participate in this study, endorsed by the Pediatric Emergency Care Applied Research Network (PECARN).

Improving The Detection of STIs in the Pediatric Emergency Department (STI)

Purpose
This study, which seeks to compare a targeted screening strategy (screening only those disclosing high risk sexual behavior) to a universally-offered screening strategy (offered to all, regardless of risk) by integrating patient-reported data into the electronic health record for the provision of clinical decision support through a multicenter comparative effectiveness pragmatic trial, will develop the evidence base for creating cost-effective and sustainable STI screening programs that can be successfully implemented in to the clinical workflow of the ED.

Understanding Vaccine Hesitancy, a Method to Improve SARS-CoV-2 and Influenza Vaccine Acceptance in Racial and Ethnic Minorities

Purpose
This is a prospective study that will utilize parent interviews and focus groups to evaluate the impact of racial and ethnic backgrounds on parental vaccine hesitancy for administration of pediatric influenza and SARS-CoV-2 vaccines. The study will seek to identify culturally sensitive interventions that will facilitate these pediatric vaccinations.

Virtual Reality Intervention (VR)

Pediatric Procedural Sedation and the Relationship with Post-Discharge Negative Behavioral Changes in the Emergency Department

Purpose
Determine if a pre-sedation virtual reality intervention will decrease pre-sedation anxiety levels and post-sedation negative behavioral changes in children undergoing procedural sedation with IV ketamine in the ED following discharge home.

Purpose
Compare the sensitivity of clean catch samples to first catch urine, an acceptable diagnostic method, and vaginal swabs, the current optimal standard, in adolescent females at risk for chlamydia and gonorrhea.
